To fill the form simply tab your way through the form, inserting the information as required. Once completed you will have two options;

1. print the form using the print button
2. email the form (using the email button). This sends only your inputted information (without the page template) to the club, to facilitate the input of client information into J&M Gymnastics Club files. The club will still require a hard copy of the form with signature. No personal information is shared with other organizations.

Registration is complete once dues have been received.
